What are “All in 4”, “All on 4” dental implants or Teeth in a day?

This is designed for people who have no good teeth left, and need them all out and new ones placed.

How is it possible ?
  • You can choose to go for just the upper teeth, the lower teeth, or both upper and lower at the same time (this is the most common approach)
  • Firstly a throughout assessment is carried out to make sure that this is the right treatment for you. Alternatives will be discussed, and usually CT-scans will be needed for a complete assessment.
  • In the morning, the remaining teeth will be extracted and the implants are placed, usually 6 in the upper jaw, and 4 in the lower jaw. We often need to use artificial bone to supplement any missing bone that you may have.
  • Then a fixed bridge is placed in the afternoon.

The history

All-on-4 is a registered trademark of Nobel Biocare, a leading brand of Dental Implants. It is a technique for replacing teeth using fewer implants than would normally be required. In particular:

  • A minimum of 4 implants per arch. Having said this, it is normal to use 6 in the upper jaw and 4 in the lower, mainly because the lower jaw is thicker than the upper.
  • The implants at the back are rotated approx 45 degrees forwards. This is beneficial as it lets the implant engage more bone than would otherwise be possible. It also avoids touching the maxillary sinus which is an air space in the jaw which the implants must not touch / would need extensive bone grafting or additional treatment if they were to touch.
  • The ability to be fitted, and restored (which means fitting the teeth which you see) in a day, i.e. ‘teeth in a day’.

Aftercare

After this treatment, you’ll probably want to take a few days off work to recover whilst getting used to your new teeth. This is why we usually suggest getting the treatment done on a Thursday or Friday.

For a few months, we suggest eating only very soft food so that the your body can heal around the implants and ‘integrate’ with them. This is the magic that makes the implants so strong – the bone of your body actually fuses to the implant so that they essentially become part of you. During this time, please don’t smoke !

Implants, although very strong, can still get gum disease, so you have to keep them clean. You brush them just like your normal teeth, two times a day. And clean using floss or a water flosser. Unsure how to use this ? Don’t worry, as part as the normal protocols we will go through all of this, step by step.

The final fit

After 9 months your implants should be completely fused with the bone and we can fit a stronger, permanent overdenture. This is custom-made, and can be made of porcelain or metal – the dentist will advise which is better for you, depending on how you bite and how you use your teeth. Please look after and clean them regularly – they should. last 10 years. You must return to your dentist for checkups.

When you choose your All-in-4 dentist, please make sure that they experienced in this treatment, because it is a very specific type of treatment and ‘technique sensitive’. This means that one of the major factors in determining how successful or unsuccessful the treatment is is the experience of the person doing the treatment.

How much do All-in-4 dental implants cost

You will need to visit our Dentist in Walsall / Birmingham for an accurate price, because the work involved in each case varies very much. More importantly, All in 4 / All on 4 dental implants are not alway possible, and other options will need to be discussed.

Having said that, the cost is from £10,000 – £14,000, per arch. This includes assessment appointments, impressions, CT-scan, bone augmentation, implant placement, temporary bridge, review appointments, permanent bridge placement including the bill from the dental laboratory, and review / adjustment appointments.

Is it worth it, compared to dentures ?

The worst thing about plastic dentures is that they usually rest on your gums. Because your gums move and change shape of time, the dentures become looser and need replacing. (You can get dentures made of metal that gain support from your teeth also- talk to your dentist).

Also some people find dentures bulky and don’t like the fact that you have to take it out to clean / whilst sleeping. They want something that stuck in a permanent like their original set of teeth.

Let’s put it this way – We can’t remember a patient who’s regretted moving from dentures to All in 4 dental implants !
